Create your first form

The Forms page opens on one question: what do you want to create? Describe the form in a sentence — "a registration form for our summer programme, with a parent contact and a photo consent" — and Matiks builds it in your workspace: the questions, the pages, and the object each respondent becomes.

Prefer to start from nothing? "Start from a blank form" opens straight in the builder.

Finding your way around the builder

• Pages live in the left panel. Each page holds its own fields, and respondents move through them in order.
• Click a field to open its settings — label, required, placeholder, and the options for choice fields.
• The Design tab controls colours, the corner radius, the progress bar and your logo. Changes preview live on the canvas.
• Results, Contacts and Workflow are tabs on the same form, so everything about one form stays in one place.
